    Community Roots specializes in residential services for adults with intellectual and developmental disabilities under the HCBS-DD Medicaid Waiver. We strongly value relationships and building deep connections and community. Most importantly, we want to build a safe, close relationship with our clients and their families/support team and spend time learning what is most important to them. We’ve developed a Person-Centered Planning process that allows us to not only get to know our clients in-depth, but to create a document that will offer life-long advocacy for each person. We recognize the negative impact that high turnover can have on our clients in residential services and we’ve built our process around mitigating this as much as possible. We offer several different residential setting options including Family Caregiver, Host Home, Live-In Caregiver, or Independent Living depending on each person’s needs.

    When you contact us, you’ll work closely with our CEO to learn your specific agency and residential support needs. Alicia will meet with you and your family/support team and determine which of our Care Managers will be the best fit to work directly with you and your team. We value open communication and encourage feedback around what is working well and how we can improve throughout the process and throughout your time working with us.

  • Supported Community Connections

    Community Roots offers individualized Supported Community Connection (SCC) services for clients in our residential program who want more than the traditional day program model. We believe everyone can benefit from one-on-one or small group community experiences and we will work with our clients to develop a schedule that captures their interests, passions, and goals. Our core value is to be fully present with our clients and to meet them exactly where they're at. SCC is a fantastic opportunity for our staff to spend quality time with and build deeper connections with our clients and support everyone to connect meaningfully with their community.
